Bequest Language
Below is language you can use to make a bequest to support our cause. A bequest allows you to continue to use the property you will leave to us during your life and make a lasting impact on our work after you are gone.
For individuals who wish to make a bequest in their Will or Trust to Cook Children's the following language is:
"I, [name], of [city, state, ZIP], give, devise, and bequeath to W.I. Cook Foundation, Inc. dba Cook Children's Health Foundation [written amount or percentage of the estate or description of property] for its unrestricted use and purpose."
If the donor wishes to restrict the bequest to a certain program or project, please also include the language below.
"I, [name], of [city, state, ZIP], give, devise, and bequeath to W.I. Cook Foundation, Inc. dba Cook Children's Health Foundation [written amount or percentage of the estate or description of property] for the purpose of (INSERT PURPOSE)."
If, in the judgement of the Board of Cook Children's Health Foundation, changed circumstances should as some future time render the designated use of this gift no longer appropriate, then the Board shall use the gift to further the objects and purposes of Cook Children's giving consideration to my special interest as evidenced by the purposes described above."
